This form provides permission for your child to be included in the group photograph only. No individual portraits will be taken unless your child is separately registered.

I am a legal parent or guardian of this child.

My child’s first name will appear on the group shot.

If there is more than one child in the group with the same first name, the first initial of the child’s surname will also be included.

The group shot will also include the centre’s logo, Five Graces Photography logo and the year 2026.

The group shot will be available to all kinder families for purchase.

The group shot will be supplied to the centre for use at their discretion (including online or display within the centre).

Five Graces Photography may use this group shot for promotional purposes including online and social media.

If this signed form is not submit before Registrations close at my centre, my child will be excluded from the group shot. Five Graces Photography will not return to photograph any children missed due to late submission of this form.
