The group shot will be available to all kinder families for purchase.*

The group shot will be supplied to the centre for use at their discretion (including online or display within the centre).*

Five Graces Photography may use this group shot for promotional purposes including online and social media.*

If this signed form is not submit before Registrations close at my centre, my child will be excluded from the group shot. Five Graces Photography will not return to photograph any children missed due to late submission of this form.*
